James Flagg’s original name is Eric. He was Mary’s abusive and possessive boyfriend, not her brother. Eric tries to kill Grace, Philip’s wife, but she manages to escape. Eric later gives up and is admitted to a mental institution. When Philip meets him there, he refers to him as “Portland.”

At first, Philip does not understand its meaning, but then realizes that Joan, Mary’s best friend, was killed on the road when she was hit by a car in the park. The incident took place in Portland. Eric killed Joan to destabilize Mary and accept him again. But he didn’t and Eric killed Mary. It was not a suicide, it was a murder. Eric is the fugitive killer. Eric escapes from the mental hospital and lashes out at Lucy, Philip’s daughter. But Philip and Grace save Lucy. Eric dies in the end of Every Breath You Take.

In the end, a metaphorical element is explored. Grace stabs Eric with her son’s ice skates. The game they were going to before the accident that killed their son.
